The impact of chronic illness extends to all healthcare settings. UniNet offers FREE disease management services in English or Spanish to adult patients with diabetes, congestive heart failure, or chronic obstructive pulmonary disease who are referred by a UniNet provider. Because these programs are funded by UniNet, services are available to UniNet providers only, and there is no charge to patients or payors.
UniNet's Disease Management programs offer telephone contact with patients for a comprehensive assessment interview to identify patient needs, and subsequent education and support through ongoing telephone calls and written material. Self-management skills and follow-through with the patient's medical plan of care is emphasized.
Providers receive written notification of the program's initial interaction with a referred patient as well as periodic updates of ongoing activity with the program.
